I know that myself and a few others have experienced this bug. Plane of Time won't progress after Phase 1 for whatever reason. I tested this today specifically since I had a decent idea of what it might be. I believe it stems from the earth trial. The earth trial is set up to spawn the named after a certain number of mobs have died in the area. If you spawn Terlok of Earth and don't kill him immediately and continue to kill the leftover mobs in the area ANOTHER Terlok will spawn. I've only experienced this bug (3 times now) when I spawn both Terloks and kill them.
